I acknowledge that participation in basketball activities involves inherent risks of injury. I voluntarily assume these risks and release the organizers, coaches, staff, volunteers, and facility owners from liability for any injury, illness, loss, or damage arising from participation, except where prohibited by law.*

Photo and Video Release

I grant permission for the organizers to photograph and record the participant during program activities and to use these images or videos for promotional, educational, or marketing purposes, including on websites and social media, without compensation.*
